VCU Medical Center Home Hospital

Inpatient hospital-level care in the comfort of your home

Your access to health care is important to us. Our Home Hospital program allows you to be treated for certain medical conditions in the comfort and security of your home instead of at the hospital.

How does it work?

Through our Home Hospital program, you will receive the same high-quality care you would receive at our medical center. Our doctors will virtually visit you with a provided mobile tablet device to check on your recovery and answer any questions. In addition, you will have at least two daily in-person home visits by our nurse and medic team. We will also check on you virtually throughout the day, assisting with your medications and monitoring your vital signs like blood pressure, heart rate and temperature.


 

Who is eligible for the program?

We will conduct a screening test to determine if our Home Hospital program is right for you.

To qualify, you must:

  • Come to VCU Medical Center’s Emergency Department with a condition or symptoms that can be safely and effectively managed outside the hospital
  • Are already in the hospital, and the remainder of your hospital care and treatments can be safely transferred to our Home Hospital program
  • Have a low risk of complications
  • Must be in stable condition and live within 25 miles of VCU Medical Center

What services are offered?

The services you receive will depend on your condition and needs. You may receive:

  • Delivery of medical supplies and equipment, like wheelchairs, walkers or bedside commodes
  • Imaging services, like X-rays and ultrasounds
  • Laboratory tests
  • Meals and nutrition services
  • Oxygen therapy
  • Medication support, including IV treatments
  • Physical and occupational therapy
  • Skilled nursing
  • Transportation (non-emergency)

You may also receive support from our care coordinators with scheduling and arranging additional services, including meal delivery, pharmacy drop-off and visiting nurse appointments.

After your discharge from our Home Hospital program, we will continue to support you through our patient navigator program.

Our patient navigator liaisons and social workers will periodically check-in with you to make sure your recovery is going smoothly and that you are getting the the follow-up care you need.
