Social Work & Care Management

The Social Work/Care Management Department at VCU Health Community Memorial Hospital is comprised of social workers, nurses and support staff with an extensive background in many clinical areas. We have specially trained staff, to help coordinate medical care for patients, serve as a patient advocate, providing information and support for patients and their families. Our services can be especially helpful for patients and families who may feel overwhelmed:

Social Work

Professional social workers are here to help you and your family with the social and emotional concerns related to your illness. Social workers are available in outpatient areas (dialysis, oncology, and emergency department) and to all residents of our long term care facility to provide support to patients and families in coping with their health care concerns. They are available to provide information and referral to community resources. Social workers are also available to provide you information related to advance directives and assistance in formulating a directive, as requested.

Discharge Planning

Discharge planning begins the day you are admitted to the hospital. Your physician, discharge planner, and nurse, work as a team to coordinate & plan for your discharge. If you require additional follow-up at home, your physician may request visiting nurses, medical equipment, rehab services or other services. Experienced discharge planners are available to assist with these services or Skilled/ long-term care placement, if needed after your hospital stay.

Utilization Review

Care Management staff is experienced in areas such as managed care and state and federal programs. The registered nurses will work with insurance carriers to determine coverage and reimbursement for your hospital stay, and work with your physician regarding your plan of care.
